Applications for Initial Accreditation
Prior to obtaining an application for initial accreditation of an educational program in Radiography, Radiation Therapy, Medical Dosimetry, or Magnetic Resonance, please contact a member of the JRCERT professional staff at: 312-704-5300.
The following is an overview of accreditation forms available. All forms with the exception of order forms are in Microsoft Word fill-in format.
Form to be completed for continuing accreditation of a radiography, radiation therapy, magnetic resonance, or medical dosimetry program - Form 100C
Form to apply for recognition of clinical education/practice setting(s) - Form 104
Form to apply for recognition of program officials - Form 102
Form to report a clinical education/practice setting status change - Form 106
Form to report reactivation and/or clinical capacity change of a clinical education setting - Form 1010
Annual clinical education setting reporting (only for Radiography & Radiation Therapy Programs in the State of California) - Form 1012
Used to assure the latest American Society of Radiologic Technologists (ASRT) or American Association of Medical Dosimetrists (AAMD) professional curriculum is implemented by the program
Forms for submission of an Interim Report.
Used by site visitors to report the results of an on-site evaluation of a radiography, radiation therapy, magnetic resonance, or medical dosimetry program
